Parent Matching Program Saving Little Hearts keeps a database of families containing information such as which heart defect or defects they have experienced, and where they received medical attention. If you would like to be put in touch with another family who has gone through a similar experience, email us and let us know. We take pride in bringing people together. If you would like to be a part of the parent matching database, you can now fill out this form online or complete this form and mail it to us or fax it to us at 865-579-5599. We respect your privacy and will not distribute your information to anyone without your consent. In addition to the information form, we will need each participant to print, sign and snail mail the HIPAA form (for CHD minors / for CHD adults) back to us before we can release your information to anyone or release any information to you. We want to make sure that each person's confidential information is properly handled.
